Grants from the Davie County Hospital Foundation Fund will be made once a year for projects that improve Healthcare in Davie County. Healthcare projects in the areas of Wellness and Prevention; Access and Awareness; and Mental Health and Behavioral Health will be considered by the selection committee.

Family Care Center of Mocksville’s Hypertension & Weight Management Program (SMBP) – self-monitored program that consists of patients’ journaling their daily blood pressure, exercise, and food intake, free monthly weight-management counseling sessions and regular check-in appointments with their healthcare provider every three months to review their progress and address any concerns – $3,104
